Combination of Effleurage Massage with Virgin Coconut Oil and Repositioning to Prevent Pressure Injury in Non-Hemorrhagic Stroke Patients: A Case Report

Abstract
Background: Stroke frequently causes hemiparesis and prolonged immobilization, increasing the risk of pressure injury due to impaired perfusion and sustained pressure over bony prominences. Preventive nursing strategies focusing on skin integrity are essential. Effleurage massage improves circulation, while virgin coconut oil (VCO) maintains skin hydration and tissue elasticity. Repositioning reduces prolonged pressure exposure.
Objective: To describe the effect of combining effleurage massage using virgin coconut oil and scheduled repositioning on pressure-injury risk in non-hemorrhagic stroke patients.
Methods: A descriptive case report was conducted in four immobilized non-hemorrhagic stroke patients aged ≥50 years. The intervention consisted of effleurage massage with VCO twice daily (10–15 minutes) and repositioning every 2 hours for three consecutive days. Pressure injury risk was measured using the Braden Scale before and after intervention.
Results: All patients demonstrated improvement in Braden Scale scores from high-risk category (10–12) to moderate-risk category (13–14). Improvements were observed in moisture, mobility, nutrition, and activity components.
Conclusion: The combination of effleurage massage using VCO and scheduled repositioning reduced pressure injury risk in immobilized stroke patients. This intervention may be integrated into routine nursing care.
